Autogenous tooth transplantation is the process of moving a fully intact tooth from a low-functioning or non-functional position to replace another tooth in the same individual.
Below is an example of autogenous transplantation where a misaligned wisdom tooth was moved to replace a lower first molar (tooth number 6) that could no longer be preserved, restoring both function and aesthetics.
A 21-year-old male patient visited due to a severely decayed tooth number 36, which had failed root canal treatment and was largely broken down. The dentist examined and took an X-ray, discovering that the wisdom tooth number 38 was still developing its root apex.


The patient chose autogenous tooth transplantation, where the developing wisdom tooth 38 was transplanted immediately into the site of the extracted tooth 36.

The patient received postoperative care, a soft diet, and antibiotics. The transplanted wisdom tooth was monitored for pulp healing.

Tooth stabilization was maintained for 1 month. During the first 3 months, monthly radiographs were taken to monitor pulp healing. The patient showed good healing and normal chewing function returned after the tooth was unstabilized.

At 2-year follow-up, the patient had completely normal chewing function, and the aesthetic appearance of the tooth was indistinguishable from surrounding natural teeth. The bite was tight and functional. The patient underwent periodontal evaluation and X-rays to assess ongoing root development.


Thus, autogenous tooth transplantation is a grafting technique capable of restoring a tooth that is histologically, aesthetically, and functionally intact. However, it is a complex procedure with strict indications for success.
