I. Causes and Symptoms of Primary Tooth Pulp Disease
1. Causes of Pulp Disease in Primary Teeth
- Untreated tooth decay significantly increases the risk of progressing to pulpitis. Tooth decay begins when bacteria in dental plaque metabolize sugar into acid, leading to demineralization. Without proper brushing and oral hygiene, bacteria continue to grow and cause cavities. Decay penetrates enamel, dentin, and reaches the pulp through dentinal tubules. The pulp contains blood vessels, nerves, and connective tissue—when invaded by bacteria, inflammation can occur.
- Trauma is a major cause of pulpitis in children. Direct impact to the teeth from accidents or falls may injure the pulp and lead to acute symptoms.
- Frequent consumption of sugary foods and drinks, especially before bedtime without brushing, facilitates bacterial growth and tooth decay.
- Genetic factors also influence a child’s susceptibility to decay and other dental issues.

2. Symptoms of Pulp Disease in Primary Teeth
- Toothache: Children often experience pain when chewing, especially at night due to increased nerve stimulation when lying down.
- Toothache can lead to loss of appetite, fatigue, and reduced playfulness.
- Clinically, cavities may appear on the chewing surface or between teeth, with large cavities containing soft, mushy dentin that are very sensitive when touched.
- Tooth discoloration: If a child’s tooth turns gray or black, this may indicate pulp disease.
- X-ray images show large cavities close to the pulp or with direct communication with the pulp chamber.

II. Treating Primary Tooth Pulp Disease Early Is Essential
- Treatment depends on the stage of inflammation and the child's overall health.
- In early-stage reversible pulpitis, treatment may involve antibiotics to control infection and reduce inflammation. This allows the pulp to regenerate and recover its function.
- In severe pulpitis with large cavities, tooth fractures, or deep bacterial invasion, pulp removal (pulpectomy or pulpotomy) may be a more effective option. This procedure eliminates infected tissue, reduces pain, and prevents further infection. After removing the pulp, the tooth is rebuilt to maintain its function.
Note: Pulpitis is a serious condition and cannot heal on its own. Home remedies are ineffective and may worsen infection and pain. Children must be examined and treated by dental professionals at specialized clinics.

III. How to Prevent Pulp Disease in Primary Teeth
- Preventing pulpitis in children helps protect dental health and avoid serious issues with gums and the pulp. Below are useful prevention tips:
- Teach children proper brushing habits from a young age. They should brush at least twice a day, for about 2 minutes each time, using a soft-bristled brush and fluoride toothpaste.
