Regenerative Endodontics: A New Advancement in Preserving Natural Teeth

For many years, root canal treatment (endodontic therapy) has been regarded as a reliable way to “save” a tooth when the pulp becomes inflamed or necrotic. However, in children—or in teeth with incomplete root development—conventional approaches often do not allow continued root maturation, which can leave the tooth structurally weaker and more prone to fracture.

This is precisely why regenerative endodontics—also known as the Regenerative Endodontic Procedure (REP)—was introduced: an advanced approach that aims to regenerate pulp-like tissue and promote continued development of the natural tooth structure from within, rather than only disinfecting and sealing the root canal system as in traditional treatment.

Suitable Candidates

Regenerative endodontics is primarily indicated for immature permanent teeth with open apices, most commonly seen in children and adolescents.
Some typical cases include:

  • The pulp has become necrotic due to extensive decay, trauma, or long-standing infection.
  • The root is still immature, with thin dentinal walls and a wide root canal space.

For mature teeth, this technique is still under active research and is currently applied only in carefully selected cases.

Mechanism and Clinical Principles of the Technique

Instead of removing the entire pulp tissue, the clinician gently disinfects the infected canal space and then induces controlled bleeding at the root apex.

The resulting blood clot serves as a biologic scaffold, allowing the patient’s own stem cells to migrate into the canal and support the formation of new pulp-like tissue.

Finally, the tooth is sealed and restored, enabling continued root development and helping re-establish internal vitality in a more natural way.

Outstanding Advantages

Compared with conventional root canal therapy, regenerative endodontics offers several notable advantages:

  • Preserves the natural tooth and the biologic function of the pulp.
  • Supports continued root maturation, increasing dentinal wall thickness and reducing the risk of fracture.
  • May enhance long-term tooth survival through the formation of new dentin and pulp-like tissue.
  • A minimally invasive approach that is generally safe and well-suited for pediatric patients.

In-Depth Discussion of the Advantages

One of the core clinical values of regenerative endodontics is its potential to preserve—or re-establish—pulp vitality, which conventional root canal therapy cannot achieve.

In conventional root canal treatment, the clinician removes the entire pulp tissue, including the blood vessels and nerve fibers within the tooth. While this effectively eliminates infection, the tooth also loses its internal blood supply and sensory feedback. Over time, it may become more brittle and susceptible to discoloration, cracking, or even root fracture due to the loss of internal nutrition and hydration

In contrast, regenerative endodontics aims to re-establish blood supply and biologic activity within the root canal space—helping the tooth remain vital, responsive, and better able to adapt to functional chewing forces. As root development continues, dentinal walls can thicken and new hard tissue may form, resulting in a tooth that is stronger and more durable than one that has undergone conventional root canal treatment.

Beyond extending the tooth’s longevity, this technique may also reduce the risk of cracking/fracture and recurrent apical inflammation—complications that are more commonly associated with conventional root canal treatment.
Over the long term, preserving tooth vitality can help maintain a more natural chewing sensation, support stable occlusion, and preserve natural aesthetics.

From a biologic perspective, regenerative endodontics does more than simply “treat” disease—it seeks to restore the tooth’s natural vitality from within, aligning with the modern dental philosophy of maximum preservation with minimal intervention.

Requirements for the Clinician and the Treating Facility

This is an advanced technique that requires the clinician to have in-depth knowledge of pulp biology, stem cell–based healing principles, and strict aseptic control.
Inducing apical bleeding and supporting new tissue formation must be performed with precise, gentle handling and a strong understanding of physiologic healing processes.
The clinic should be equipped with modern endodontic systems, biocompatible materials, and a standardized sterile environment to support stable, long-term outcomes.

Regenerative Endodontics at Herident

At Herident Dental System, regenerative endodontic procedures are delivered under the clinical oversight of our endodontic and pediatric restorative team, following strict aseptic protocols and a tooth-preservation philosophy that prioritizes maximum conservation of natural tooth structure.

As a result, many young patients are able to retain their permanent teeth, avoid premature extraction or early restorative replacement, and maintain natural aesthetics and chewing function for years after treatment.
