Initial Condition of the Patient
Mr. N.H.Đ, 26 years old, visited Herident with existing oral health concerns. Although he was aware of his condition, he had delayed treatment for years.
Initial Diagnosis:
Through clinical examination and X-rays, doctors diagnosed him with serious complications caused by postponing wisdom tooth extraction. Specifically:

1. Examination and Consultation
- Initial clinical assessment: A general evaluation of the patient’s oral condition and wisdom tooth positions was performed. In this case, the patient had all four third molars. Teeth 38 and 48 were mesioangularly impacted, while teeth 18 and 28 lacked occlusal contact and were over-erupted, causing food impaction and hygiene difficulties.
- The patient was given a panoramic X-ray to determine tooth angulation, jawbone condition, and the impact of the wisdom teeth.

On the panoramic film, teeth 38 and 48 were located close to the mandibular nerve canal. However, as the panoramic film is only 2D and the nerve canal borders appeared visible, the doctor ordered a CBCT scan to ensure safety by visualizing the relationship between the tooth roots and the mandibular nerve in 3D.

Tooth 38 had two curved roots, was not in contact with the nerve canal, but was tilted mesially at a 70-degree angle, impinging on the distal cervical area of tooth 37 and causing deep caries near the pulp.

Teeth 48 was mesioangularly impacted at an angle of 47.78 degrees, with two curved, hook-shaped roots, close to the mandibular nerve canal, and showed bone loss on the distal side of teeth 47.
- Based on clinical and paraclinical examinations, the doctor indicated the extraction of all four wisdom teeth, with the extraction plan divided into two sessions:
Session 1: Extract tooth 28 and 38. After about 2–3 weeks of healing, proceed to restore tooth 37 with deep caries at the distal cervical area.
Session 2: Extract tooth 18 and 48.
As the patient was afraid of swelling and pain during wisdom tooth extraction, in this case, the doctor used the extraction method combined with piezotome to reduce trauma and postoperative swelling.
2. Pre-extraction preparation

5. Post-extraction instructions
- The doctor prescribed painkillers, anti-inflammatory medications, and antibiotics.

- Scheduled a follow-up appointment after 3 days to check the wound and monitor healing.
